Directory Information: Prior Written Notice
South Valley Academy may publicly disclose certain Personally Identifiable Information (PII), known as directory information, from your student's educational records without prior written notice. This disclosure occurs for the purposes of generating school publications such as the yearbook, athletic rosters, school email addresses, etc. Please review the full Directory Information Annual Notice, linked above, for a full list of educational records which are considered to be directory information.
If you would like to request that South Valley Academy notify you in writing prior to the disclosure of any directory information, please provide written notice to our main office at this address:

Requests for prior written notice in regards to Directory Information must be made by the end of September each school year.
Health Insurance Portability and Accountability Act of 1996 (HIPAA)
South Valley Academy students may receive healthcare or therapeutic services via a third-party, non-SVA, contractor while on campus. These transactions and the transmission of electronic information for billing purposes may be covered under HIPAA.
